If the death has just happened, do this first
- At home with hospice? Call hospice first. A nurse must pronounce the death before anyone can be moved.
- At home unexpectedly? Call 911 first. The medical examiner has to release your loved one before we can help.
- In a hospital or care facility? Tell the nurse you have chosen us. They will hold your loved one until we arrive.
You can start this form at the same time. We will coordinate the timing on the call back.
Begin the arrangement
Nothing here commits you and nothing is owed. Submitting this does not authorize a cremation — that requires a separate signature from the person with the legal right to authorize, and we walk you through it afterwards.
Veterans: cremation with honors
We file for the VA burial allowance — up to $2,000 for service-connected deaths, $948 otherwise — and arrange DFW National Cemetery committal, where there is no charge for the plot. Tell us your loved one served and we handle the filings.
